Saint Vincent College Athletic Event/Tournament Release, Waiver of Liability, and Indemnity Agreement: Minor/Participant

I/We, the parent(s)/guardian(s) of participant [hereinafter Participant], identified below, hereby acknowledge that the participant is being offered the opportunity to engage in an athletic event/tournament, (hereinafter “the Activity”) specifically SVC Football Elite Prospect Camp to be held on the campus of Saint Vincent College on Friday, May 15, 2026.

I/We hereby acknowledge that any reference to Saint Vincent College in this document includes the Saint Vincent College Corporation, the Saint Vincent Archabbey, the Benedictine Society of Westmoreland County, Saint Vincent Seminary, the Wimmer Corporation and any and all past, present and future governing bodies, members, managers, directors, officers, employees, and agents of the aforesaid entities, including their insurers, employee benefit plans and their respective trustees and fiduciaries, and all others acting in concert with them.

I/We understand and agree that Participant’s participation in the Activity is strictly voluntary.

I/We understand and acknowledge that the Activity involves competitive events and that Participant could be injured if he or she participates.

I/We agree to instruct Participant to comply with all safety precautions andinstructions from all authorized personnel in connection with the Activity.

I/We voluntarily assume and all risks of injury to Participant caused by, or arising from his/her participation in the Activity.

I/We understand and acknowledge that as a precondition to Participant’s participating in the Activity, Saint Vincent College is requiring us to release it and indemnify it against any loss, damages or costs, from any claims or causes of action that I/We might have against Saint Vincent College arising from our child’s participation in this Activity.

I/We understand that by signing this release I/We are voluntarily giving up legal rights that I/We might have against Saint Vincent College as are set forth below.

I/We knowingly and voluntarily indemnify, release and hold harmless Saint Vincent College, from all claims relating to, or arising from Participant’s participation in the Activity including, but not limited to, any personal injury claims or claims of general or gross negligence.

I/We further acknowledge that Saint Vincent College does not provide any supervision, instruction or assistance in regards to the above mentioned Activity unless it has separately agreed to do so in writing.

I/We hereby agree that Saint Vincent College is not responsible for any property that is lost, stolen, or damaged while in, on, or about the premises of the College during the Activity.

I/We hereby represent to Saint Vincent College that Participant is physically fit and capable of participating in the above mentioned Activity; that he/she has had a recent physical examination by a competent physician and that his/her participation is consistent with all medical advice I/We have presently received from medical professionals familiar with his/her physical condition; and that I/We and Participant have inspected the facilities prior to their use.

I/We further represent to Saint Vincent College that Participant is covered by some form of hospitalization, accident, automobile and/or other property or liability insurance, that such insurance will be applied toward the payment of any loss, injury, or damage and that I will not look to Saint Vincent College for the payment of any such loss, injury, or damage or any expense or cost related to it.

I/We further expressly agree that this release, waiver and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of the Commonwealth of Pennsylvania and that if any portion thereof is held invalid, it is agreed that the remainder shall, notwithstanding, continue in full legal force and effect.

Intending to be legally bound hereby, I/We have read the foregoing waiver and release of liability and indemnity agreement and voluntarily agree to execute this document with full knowledge of its contents.

Saint Vincent College Photo/Promotional Material Release Form

I/We are the parent(s)/legal Guardian(s) of the participant [hereinafter Participant], identified below, who is to participate in SVC Football Elite Prospect Camp (hereinafter “the Activity) at Saint Vincent College on May 15, 2026.
 
I/We acknowledge that during the camp Saint Vincent College may take photographs of Participant and their activities to be used in marketing promotions and other publications sponsored by the College. I understand that these photographs will be used for educational purposes and not for the commercial gains of any individual.
 
I/We irrevocably grant to Saint Vincent College the right to use any photograph of Participant that is taken during the Activity. By this grant, I specifically mean the following:
 
1.    Saint Vincent College will have total ownership of the program, throughout the world, in perpetuity, including, without limitation, the right to edit, broadcast, and make other use of the program or excerpts thereof in any medium, the right to copyright the program or edited materials, and the right to license others to utilize these rights.
 
2.    Saint Vincent College may use Participant’s name, likeness, voice, biographical information, and any material supplied by me for purposes of advertising, publicity, sales promotion and institutional promotion, but not as a direct endorsement of any product or service.
 
I warrant that Saint Vincent College use of Participant’s appearance in all types of video, photo and/or audio, and/or the material provided by me as described above will not violate the rights of any person or entity and will not cause Saint Vincent College to incur any liability for payment to any person or entity.
 
Further, I expressly release Saint Vincent College, its governing boards, officers, agents, employees, licensees and assigns from any liability for any violation of personal property or rights which I may have, arising out of production, duplication, broadcast, or exhibition of my child’s appearance, voice, and/or the material provided by me, including, without limitation, liability for invasion of privacy or defamation.
 
I hereby affix my signature, attesting to my understanding of the above release, and as an acceptance of and agreement with the stated provisions.  Further, I attest that my signature is an act of my own free will, affixed with no indication of coercion nor duress.

INTENDING TO BE LEGALLY BOUND HEREBY, I HAVE A FIXED MY SIGNATURE ON THE DATE SET FORTH BELOW.
